A Health Care Surrogate gives someone else the power to make your medical decisions should you be unable to do so yourself. It also specifies what medical actions are to be taken in certain health care situations.
This is one example of an Advance Care Directive. Advance care directives are authorizations for another person to act on your behalf in a legal, Health Care, or business situation. There are three types of advance care directives: Power of Attorney, Health Care Surrogate, and Living Will.
